You can change the date to this year before you submit. This will make sure that our nurses have the updated and current health information for your child. Some of the boxes are pre-filled so we just ask that you review and and make changes if necessary and then sign and date. 𝓣𝓱𝓪𝓷𝓴 𝔂𝓸𝓾!

Reminder of Back to School Celebration tomorrow night - https://5il.co/2t6uo It is at the High School Gym to start with @ 4:30 PM, then at 6 PM there will be some teacher recognition, followed by the Rocketaires, Dance Team, Band and Cheer. Then at 6:40, everyone can go to their buildings to meet their teachers.
