A leave request form MUST be filled out and submitted to Human Resources to be approved for paid extended sick leave. Under certain qualifying reasons documentation must be provided. See the form for more details. If you are going to be tested and/or are on quarantine please do not personally hand fill out and bring the request form to Central Office. If your test comes back negative your illness is no longer considered a COVID related absence and you will be required to use your available leave. If you do not have access to fill out request form online to send back through email or fax, call Human Resources at 316-776-3300, or your Department Director, and have them fill out the form for you.
