Finding a Network Doctor, Urgent Care or Facility
Your medical plan uses the United Healthcare Choice Plus Network. If you choose to receive your medical care from a provider who does not contract with the United Healthcare Choice Plus network, you will be responsible for a higher deductible and coinsurance amounts. In addition, your benefits will be based on an “allowed” amount which is similar to the amount received by a contracting provider. A non-contracting provider can balance bill you the difference between the “billed” and “allowed” amount. This difference could be substantial.
TO LOCATE A PROVIDER:
1. Go to myuhc.com and Under “Find a Doctor”, Click “Find a Provider”
2. Select “Medical Directory” and then “All United Healthcare Plans”
3. On the “What plan are you looking for?” page select “Choice Plus”
4. Change your location at the top, if needed, and then you can search by people, places, services etc.
United Healthcare DESIGNATED NETWORK: You receive the most benefit when you use In Network providers. You can save even more money when you use Premium Care Physicians that are in the Designated Network. Doctors who have met the criteria for quality and/or cost efficiency will be labeled as Tier 1 and the Designated Network copay would apply. These providers will be noted in the Provider Directory with two blue hearts. When you do a provider search, you can filter by the Premium Care Physicians. On the left side, under Preferred Providers, click the two blue hearts—This is the designation for Premium Care Physician.
